When you reach out to Sarah Grace Home Care for the first time, our team will ask a few questions to understand your loved one's situation, what kind of support is being considered, and what's prompting the inquiry now. There's no pressure or sales pitch, just a conversation to understand your family's needs.
The In-Home Visit
Next, we schedule a free, no-obligation visit to your loved one's home. During this visit, we take time to understand daily routines, preferences, and any specific health or safety concerns. This is also an opportunity for you and your loved one to ask questions and get a feel for who we are.
Building a Care Plan
Based on what we learn, we put together a care plan tailored to your family's specific situation, whether that means a few hours of companionship each week or more comprehensive daily support. We'll also talk through payment options at this stage, so there are no surprises.
Getting Started
Once a care plan is in place, we match your loved one with a caregiver suited to their personality and needs, and introduce them during the first visit. From there, our team stays in touch to make sure the relationship is working well, and we're always just a phone call away if anything needs adjusting.
